To access your 1095-C:
- Log in to my.AState
- Select the Banner Self Service link
- Click the Employee link
- Click Tax Forms
- Click on 1095 Employer Provided Health Insurance Offer and Coverage Statement
- Select the Tax Year
- Select the Display button
If you are no longer employed by Arkansas State University, you can access past 1095-C statements here.
To print your 1095-C:
- Click the Printable 1095-C button at the bottom of the screen
- The Print 1095-C Verification screen will display. Please enter your 6 digit Banner Self-Service PIN. Your full 1095-C will be displayed.
- Click on File>Print
You can always view and print historical 1095-C statements by changing the Tax Year.
